To make the most of your clinic
visit, please bring the following personal
medical information with you to the
appointment:
- Completed New Patient Forms
- Insurance information
- Referral from your primary care
physician (PCP) when required
- Previous operative reports
- Actual films and reports of any
previous x-rays or MRI scans
- List of allergies and any
medications you are taking
- Previous medical records related to
your condition
Check In
Take the main elevators to the 20th Floor
of the Memorial Hermann Medical Plaza Building. At
the reception desk, we will verify your
personal information, referral
authorization, and collect your co-pay.
Dr. Smith’s Exam
Dr. Smith will take a thorough history
and perform a detailed examination. During
the visit, please feel free to ask
questions. After the exam, you will receive
a clear explanation of your diagnosis and
the reasons for any additional tests.
Treatment options will be discussed and we
encourage your participation in planning the
course of care.
