Please arrive 15 minutes prior to your scheduled appointment time to complete the necessary forms and insurance verification. :

To make the most of your office visit, please bring the following to your scheduled appointment:

• Your insurance card
• Referral from your primary care physician (PCP)
• Previous operative reports
• Actual films and reports of any previous x-rays or MRIs
• A list of current prescription and/or over-the-counter medications you are taking (please include your dose and frequency)
• Registration and history forms - available through the Forms section of this website
